**mecwa**_care_ is seeking a stand-out Disability Liaison Officer to ensure customer centric work practices are in place. This role will facilitate the provision of effective customer engagement and administrative support from initial intake prior to handover to the service delivery team. Responsibilities include facilitating access to NDIS services, developing care plans with participants, and completing assessment paperwork for service access and annual review as well as building networks and promoting community engagement.

**What you bring**
- Certificate IV in Disability or equivalent
- First Aid level 2 Certificate and NDIS Check
- Rostering and scheduling experience in aged and disability service environment.
- Demonstrated experience in a client focused position with a commitment to providing superior customer service to the most important members of the team - our clients and our staff.
- A logical mind with an ability to see the bigger picture - rostering is a puzzle waiting to be solved.
- Strong administrative and organisational skills with a high level of attention to detail.
- Strong IT skills including Microsoft Office suite.
- Knowledge of Carelink+, CHSP, HACC guidelines and My Aged Care Portal.
