- Communicate with Health Practitioners, and update staff and families of changes in participant health.
- Provide monthly reports on Participant Health & Wellbeing and run timely MedSig reports.
- Mentor and build confidence in direct support workers to develop the skills and understanding they need to improve their practice supporting participant’s health and wellbeing.
- Maintain accurate and comprehensive client records and documentation, ensuring all information is up-to-date and compliant with relevant regulations.
- Build capacity in individuals to manage their own supports.
- Ensure all services comply with relevant legislation, policies, and procedures.
- Advocate for the rights and needs of individuals with disabilities.
- Stay informed about industry best practices and developments in disability services to continuously improve service delivery.
