**About the Role**:
The Client Liaison officer provides business development opportunities, offers marketing initiatives, seeks out referrals and works closely with the Care team to ensure all prospective participants referred to Serenity Home and Community Care receive efficient and timely access to appropriate support services.
The position will liaise with relevant external stakeholders and progress the collating and prioritizing of relevant documentation to facilitate securing the participant referrals.
**Key Responsibilities**:
- Manage the intake process for new participants seeking disability support services.
- Conduct an initial evaluation of participants to ascertain their specific support requirements and objectives. Such assessments include diagnosis, functional capacity, mobility, modes of communication, cognition, self-care and management options.
- Assess what funding arrangements are in place for the PWD to support them with the appropriate NDIS options available.
- Complete onboarding of new participants with all relevant and compliant information
- Including Participant Profiles, Care Plans, Risk assessment, Service Agreements, Schedule of Supports, medication profiles etc.
- Liaise with participants, their families/carers, and other stakeholders to coordinate the intake process.
- Maintain information and data in an accurate and compliant manner.
- Working closely with the team to identify the appropriate support workers for clients with unique and complex needs.
- Have a consistent and timely response to referrals and enquiries.
- Collaborate with the team as required to ensure the best outcomes for participants.
- Provide coverage with support of services within the Care Support team as directed.
- Create business development opportunities for new participants via marketing initiatives on social media and within the community.
- Assist with the development and implementation of activity-based group programs to increase social connectedness.
**Qualifications/ Requirements**:
- Minimum of a diploma-level education required in health, disability or community service sectors.
- Current professional networks that maintain and enhance industry knowledge and learning
**Experience**:
- Experience in managing the delivery of disability support services
- Experience working within NDIS principles and related legislative and regulatory mechanisms, responsibilities, rules and processes.
- Experience working with NDIS Practice Standards and related guidance.
- Emerging knowledge around best practice models and frameworks in disability and related services.
- Knowledge and requirements linked to National Child Safe Standards and state requirements.
- Working with participant groups who may be more vulnerable to heightened risk and strategies to remove or reduce risk.
**Skills**:
- Proven Track Record: Demonstrated success in managing the delivery of Health and Human Services with measurable achievements and outcomes.
- Disability Support Expertise: In-depth understanding of the needs of people with disabilities, with a commitment to a person-centered approach and positive behavior support.
- Team Collaboration: Strong ability to work effectively within a person-centered team, contributing positively to collective goals and outcomes.
- Leadership Excellence: Experience in leading teams to foster a positive workplace culture while effectively managing performance, complaints, and concerns.
- Strategic Execution: Proven capability to deliver on strategic objectives and operational goals, with a strong focus on customer service and community engagement.
- Critical Thinking and Problem Solving: Skilled in analyzing complex issues and implementing practical, effective solutions.
- Effective Communication: Outstanding ability to produce high-quality written reports, policies, action plans, and other essential documentation.
- Interpersonal Mastery: Exceptional skills in building credible relationships, negotiating, and resolving conflicts while maintaining a professional and respectful approach.
- Community Engagement: Proven ability to develop and maintain positive relationships with participants, families, carers, stakeholders, and team members, ensuring clear communication and collaboration for the best possible outcomes.
