Key Responsibilities:

- **Client Support**: Work closely with NDIS participants to understand their goals, preferences, and needs.
- **Plan Implementation**: Assist participants in implementing their NDIS plans, ensuring they access appropriate services and supports.
- **Service Coordination**: Liaise with service providers to coordinate and facilitate the delivery of services, ensuring timely and effective support.
- **Resource Management**: Help participants manage their funding, including budget tracking and resource allocation.
- **Advocacy**: Advocate for the rights and needs of participants, ensuring their voices are heard in all aspects of their support.
- **Monitoring Progress**: Regularly review and assess the effectiveness of supports, adjusting plans as necessary to meet changing needs.
- **Documentation**: Maintain accurate records of participant interactions, service agreements, and progress reports.
- **Community Engagement**: Foster connections within the community to enhance participants’ social inclusion and access to resources.

Qualifications:

- Relevant qualification in social work, community services, or related fields.
- Knowledge of the NDIS framework and disability services.
- Strong communication and interpersonal skills.
- Experience in case management or support coordination is preferred.
- Ability to work independently and as part of a team.

**Skills**:

- Excellent organisational skills and attention to detail.
- Problem-solving skills and the ability to adapt to changing situations.
- Empathy and understanding of the challenges faced by individuals with disabilities.
- Proficiency in using software and tools for documentation and communication.

Working Conditions:

- Typically office-based with some travel required to meet clients and service providers.
- 9am-5pm (mon to Fri)
