**THE OPPORTUNITY**

The Support Coordinator supports our clients to understand and implement the funded supports in their plan and link them to community, mainstream and other government services in order to achieve their goals. The Support Coordinator will focus on supporting clients to build skills as well as connect them to providers.

**ABOUT THE ROLE**

The key responsibilities include:

- Work with Clients to achieve their goals in line with their NDIS plan
- Engage and coordinate services providers on behalf of clients and link clients with relevant providers
- Meet with and engage with clients to develop and refine their goals
- Provide information and support to clients and their families/carers to monitor the use of their funds
- Assist NDIS participants with the plan review process
- Develop strong networks within NSW community to enhance Konnect Care presence and awareness

**Requirement**:

- At least 12 months experience in a Support Coordination Role, LAC, NDIS Planner or similar are required
- Qualifications in Community Services, Allied Health, or a related discipline (or working towards one) is highly regarded but not essential
- Effective verbal and written communication and ability to prepare clear and accurate reports and correspondence that are appropriate for the purpose and audience
- Ability to plan and manage own workload in a complex environment
- High level problem-solving abilities, as well as the ability to work effectively under pressure
- Strong inter-personal skills and the ability to communicate effectively with a diverse range of people and organisations
- A current Australian driver’s licence
- Working with Children Check and NDIS Worker Check (or willingness to obtain)

**Salary**: $58,343.00 - $99,427.00 per year
