**About the role**

The Manager will lead the Transition Care Program (TCP) and advise the Deputy Director of Operations in matters pertaining to the delivery of the TCP. The Manager will provide efficient operational management and effective coordination of the services, including human and financial aspects and brokered services. The Manager will demonstrate expert knowledge in ensuring safe, effective, contemporary and efficient clinical service delivery and will understand the essential role of effective clinical and professional governance. They will collaborate with all relevant stakeholders to achieve these objectives. The Manager is expected to possess highly developed skills in operational management, leadership, quality assurance and improvement. A key aspect of the management of TCP will be building strong, positive, robust relationships with internal and external key stakeholders and delivering service excellence.

**About you**

To be successful in this role you will possess:

- Demonstrated experience in leading and facilitating sustained continuous improvement of interdisciplinary health services
- Demonstrated operational management skills, including budget management and procurement of contracted services.
- Demonstrated ability to build and maintain effective relationships with key stakeholders
- Post-graduate level qualification in management or commencement thereof (desirable)
- Healthcare workers are strongly recommended to follow COVID vaccination recommendations provided in the _Australian Immunisation handbook (based on ATAGI advice)_. Seasonal vaccination against influenza is a mandatory requirement of this role and an offer is conditional on this being up to date prior to employment._
