**Clinical Coordinator - Transition Care Program**

**About the role**

The Clinical Coordinator applies their highly developed clinical skills and effective communication to provide professional leadership to the Transition Care Team. Using advanced clinical and professional expertise, managerial, governance, problem solving and leadership skills, the role provides high-level consultation within the team, organisation and externally contracted providers to ensure the promotion of professional values and high standards of service provision to our 94 TCP clients.

**About you**

To be successful in this role you will possess:

- Excellent communication and organisational skills
- For nursing applicants, current registration with AHPRA
- For allied Health applicants an accredited bachelors degree or entry level masters' degree in a relevant Allied health profession
- Demonstrated clinical skills aligned with experience and scope of practice
- Highly developed interpersonal and leadership experience
- A strong commitment to high quality care
- The ability to work as part of a diverse team
- Healthcare workers are strongly recommended to follow COVID vaccination recommendations provided in the _Australian Immunisation handbook (based on ATAGI advice)_. Seasonal vaccination against influenza is a mandatory requirement of this role and an offer is conditional on this being up to date prior to employment._
