Multicultural Care is looking to recruit an experienced **NDIS Care Advisor **to work within our team in coordinating and growing Multicultural Care’s **National Disability Insurance Scheme (NDIS)**. We are looking to recruit someone for this position immediately to start working with us in our new offices in Burwood

**The Role**

This role is responsible for coordinating and growing MC’s NDIS program as well as actioning referrals, liaising with NDIS participants and their families and key stakeholders whilst coordinating the delivery of NDIS supports with the assistance of the Roster Coordinators.

This Includes

**Participant Support**

Ensure participants receive support that is appropriate to their individual interests, goals, capabilities, and support needs.

Meet with participants and their support network to discuss their goals, needs and talk through the services the participants would like to access

**Service Co-ordination**

Ensure services are developed and delivered in line with funding body requirements and participant support plans, and that support and services are delivered efficiently and effectively.

**Support Coordination**

Assist NDIS participants in navigating the National Disability Insurance Scheme (NDIS) and implementing their NDIS plans

Work closely with participants to understand their individual goals and needs, connecting them with appropriate services and supports to enhance their independence and quality of life.

Involve a blend of direct participant interaction, administrative duties, and liaising with service providers.

**Document and Reporting**

Monitor and evaluate the delivery of support and services and participant outcomes using an evidence-based approach.

Ensure all financial records and required documents as per NDIS Standards for each participant are accurate and processed within specified timeframes

Ensure all activities comply with NDIS guidelines and organizational policies.

Prepare reports and documentation as required for NDIS reviews and audits.

**About You**

This is a crucial role with a high-level community partnering through frequent engagement, communication, and evaluation of programs. To be successful in this role you will have:
**Essential**
- Min, 3 years extensive experience as NDIS Service Coordination and/ or Support Coordination in the disability care framework.
- Have a thorough understanding of the NDIS, including eligibility, planning processes, funding structures, budgeting process and available support.
- Have solid experience in case management and support coordination as well as collaborating with Allied Health Professionals.
- Strong participants focus and an ability advocate for participants and their jobs.
- Cultural Competence - Have an ability to work effectively with clients from diverse backgrounds and understanding cultural sensitivities.
- Problem-Solving Skills - ability to identify and resolve issues related to service provision, client needs, and support coordination.
- Teamwork - ability to work collaboratively with other professionals, service providers, and the NDIS to coordinate comprehensive support for clients.
- Flexibility and Adaptability - ability to adapt to changing circumstances and client needs, demonstrating flexibility in approach and problem-solving.
- Ethical Practice - commitment to ethical practice, including maintaining confidentiality and acting in the best interest of clients.
- A genuine commitment to providing high quality, and disability care, and to the values and philosophies of MC.
- Good administrative and computer skills
- Driver’s licence and access to own vehicle

**Desirable**
- Hold a degree or diploma in Social Work, Psychology, Occupational Therapy, Disability Studies, Community Services, or a related field.
- Ability to speak a second language

Pay: $84,000.00 - $90,000.00 per ye
