About You
Do you have passion to assist those with a mental illness to create connections and link with sustainable supports within their community? Proven experience working or lived in Mental Health or AOD supporting those who demonstrate challanging behaviours as a result of mental health issues or cognative disability. Are you reliable and consistent in your practice?

**Additional attributes we are looking for include**:

- Ability to work both autonomously and within a team environment
- Effective listening skills & an adaptable communication style
- Capable to advocate for others and build support networks
- Ability to contribute positively within a team, work collaboratively and share idea's
- Ability to facilitate group work and activities
- Experience in building rapport with difficult to engage consumers
- Experience and or knowledge within case management
- Prior worked or lived experience supporting people with dual diagnosis, Mental health and or AOD encouraged to apply

It is a requirement of this position to be fully vaccinated against COVID-19 and meet all compliance requirements in relation to NDIS Work Screening, Working With Children’s Check, work rights and possess a current & valid driver’s license.

About the Role
The SAVVI (Supporting Accommodation for Vulnerable Victorians Initiative ) Supporting Connections Program aims to address resident unmet health and social needs and link residents to appropriate services. It also works with SRS staff and proprietors to build their capacity to meet the needs of the residents and improve their support planning to meet those needs and better manage challenging behaviours. The program also aims to work with health and local support services to improve to improve their accessibility for residents of these SRS and their responsiveness to proprietors and staff. Our success will enable them to have the voice and choice to thrive in their local communities.

**The role entails**:

- Provide outreach and case management to identified participants
- Identify participant health needs and support participants to access suitable services to meet needs
- Supporting participants to engage in both social and recreational activities
- Working with proprietors to identify participant needs, and develop skills to suitably respond to challanging behaviours
- Increase self-management and independence for participants
- Develop and maintain professional relationships with participants and all relevant stakeholders
- Providing accurate and timely case notes including development of program narrative reports and relevant documentation
