**About the Program**
Insync is a program for young people who are at risk of, or who are currently experiencing, homelessness in Brisbane, Redlands and Logan. Our team supports young people with crisis and transitional accommodation, while helping to increase their living skills and achieve their goals through a case planned process.

**About the Role**
As a Practitioner, you will provide essential case management support to young people experiencing homelessness in crisis and transitional housing. Collaborating with Youth Workers, you'll create personalized support plans and conduct follow-ups to ensure clients maintain their wellbeing and stability after leaving services. Building strong relationships with young people, colleagues, and external agencies is crucial, as you'll develop referral networks to enhance the support available to youth and empower them on their journey toward independence.

Our service is enhancing our immediate needs accommodation, transitioning to a 2-worker model with active staff around the clock, supporting young people at risk of homelessness including those with high and complex needs. Therefore availability to complete evening/night shift is required.

This role is a Full-Time 14-Month Fixed-Term Contract working 38 hours per week at our Underwood service. Contract ending December 2025 with possibility of extension pending the extension of Government funding.

Pay Rate: $49.28 per/hr - SCHADS-CAE Award Level 3

**Responsibilities**:

- Provide trauma-informed case management and support to youth in both short-term and transitional accommodation.
- Assist clients in accessing housing, health, and social support services to meet their needs.
- Maintain accurate client records and ensure compliance with quality assurance standards.
- Foster relationships with housing providers to facilitate client referrals.
- Participate in interagency meetings and represent the service effectively.
- Contribute to a positive team environment, sharing insights and supporting colleagues.

**Qualifications, Credentials and Other Requirements**
- Degree in Social Work, Human Services, or a related field, or equivalent experience.
- Current National Police Check
- Current Blue Card (Working with Children).
- Valid Queensland Driver Licence and reliable vehicle.
- Willingness to work outside standard hours and provide on-call support as needed.

**About You**:

- Strong understanding of youth homelessness and its influencing factors.
- Experience in trauma-informed, solution-focused case management.
- Excellent relationship-building and communication skills to engage with diverse stakeholders.
- Familiarity with public and community housing systems.
- Ability to work collaboratively in a team-oriented environment.
