This role is essential within the Services Team, providing assessment, care planning, monitoring, and referrals for all clients. The Support Coordinator helps participants enhance their knowledge and experience, aiding them in making connections within their community. They also assist with referrals and accessing additional support systems.

This is a casual role with potential to full time permanent.

**Key Responsibilities**:
**1. Client and Stakeholder Engagement**:

- Implement strategies to help participants build capacity and achieve their plan goals.
- Coordinate and assess supports and service providers to meet specific needs and goals.
- Build the capacity of NDIS participants and their families to navigate local service systems.
- Monitor budgetary requirements for Support Coordination.

**2. External Engagement and Networking**:

- Participate in forums, networking, and new business activities to enhance the organization's reputation.
- Adhere to NDIS Code of Conduct and Quality and Safeguarding guidelines.
- Exceed customer service expectations and comply with policies and procedures.
- Engage with healthcare professionals and community organizations to ensure coordinated care.

**3. Documentation and Compliance**:

- Complete documentation to comply with policies.
- Work within legislated NDIS frameworks and adhere to safety regulations.
- Contribute to quality processes and manage client feedback.

**4. Professional Development**:

- Actively engage in professional development activities.
- Develop and maintain relationships with community organizations for holistic case management.

**5. Business Development**:

- Manage relationships with referral sources and source new referrals in the community.
- Retain existing consumers and achieve KPIs related to new and existing consumers.

**Requirements**:

- Strong time management and attention to detail.
- Outcome-focused with exceptional communication and planning skills.
- Problem-solving ability with determination and innovative thinking.
- Customer service, relationship management, and business development skills.
- Adaptable to changing operational requirements.
- Minimum 1-year experience in the NDIS sector.
- Advanced knowledge of the National Disability Insurance Scheme and Quality & Safeguards Standards.
- Vietnamese speaking preferably.

**Job Type**: Casual

**Salary**: $40.00 - $45.00 per hour

Expected hours: 20 - 30 per week

- NDIS Screening Clearance (preferred)
