**NDIS Support Coordinator**

At ASC we are a passionate about providing exceptional support coordination services to our clients. We currently have a fantastic opportunity for 2 Support Coordinators

with similar values to join our team.

You will be working with a manageable caseload of NDIS participants to support them to build capacity and implement supports to help them achieve their goals outlined in the plans. You will be joining a highly experienced and supportive team.

**To be successful in this role, you will need**:

- Excellent communication skills, both written and verbal and the ability to communicate in a positive and collaborative way
- Effective time management skills
- Experience with keeping case notes and excellent report writing skills.
- Ability to build strong working relationships and establish an ongoing rapport with clients and stakeholders
- Professional and/or lived experience with disability
- The ability to work autonomously and efficiently from home office
- A mature, flexible and committed attitude to your clients and work
- Results driven, solutions focused
- Support coordination/disability experience advantageous but we also provide training
- You will be system savvy and comfortable with following policies and procedures
- Reside in the Eastern suburbs of Melbourne

**Preferred**:

- A relevant qualification in Community/Social Services, Allied Health, Mental Health, aged care highly regarded
- Demonstrated knowledge and experience of the NDIS

**Mandatory**
- Current Driver’s License and own car, Police check, Working with Children’s Check DWES

**Location**:

- Melbourne Eastern Suburbs VIC (Must reside in this area)

**License**:

- Drivers (required)
