**About the role**
The Health Promotion Practitioner position is responsible for the planning, evaluation and delivery of primary prevention projects across the Yarra Ranges. The Grade 2 Health Promotion Practitioner reports to the Senior Health Promotion officer and works with a small team planning, delivering and evaluation the strategies outlined in Eastern Health’s annual Health Promotion action plan. The Health Promotion Practitioner position has a portfolio of prevention tasks as agreed to in coordination with the Senior Health Promotion Practitioner and the rest of the team. The Grade 2 position also has additional responsibilities such as supervising junior staff members or students as directed by the Senior Health Promotion Practitioner. The position has a high degree of autonomy and involves working with a broad range of stakeholders and partners both internal to Eastern Health and external.

**About you**
To be successful in this role you will possess:

- Demonstrated experience working as a Health Promotion Practitioner
- A tertiary qualification in Health Promotion, Public Health or another relevant qualification
- Excellent communication and organisational skills
- The ability to work independently and as part of a diverse team
