About the Role
- Demonstrate a consumer centred approach in the assessment of diversion or recreation therapy needs
- Liaise and collaborate with the local community recreation agencies, government departments and volunteer bodies
- Prepare reports as required, maintain documentation related to Individual Care Plans and maintain administrative and data collection as required
