This is an exciting opportunity for you to become a key member of ONCALL’s dynamic In-Home Support team.

As a Service Coordinator you will be a key driver in the provision of flexible and tailored supports to people with disabilities. Your ability to engage with clients and their families, developing strong working relationships is paramount in this role.

**Key responsibilities will include**:

- Providing person centred services that enrich the lives of people living with disabilities.
- Engaging clients by understanding their needs and ensuring their profiles and related documentation are maintained.
- Actively engage with NDIS participants/families, ensuring that all key stakeholders have a clear understanding of their individual funding.
- Working with clients to ensure their teams include well matched Support Workers and that changes to their rosters are actioned in a timely manner
- Developing and maintaining professional relationships with NDIS participants/families, staff and other key internal and external stakeholders.
- Visiting clients to assist in developing care requirements and providing written reports
- Efficient and effective management of participants funding.

**Our Successful Applicant will ideally have**:

- Experience in a fast paced customer service/call centre role
- Strong interpersonal skills, enabling effective communication with our NDIS participants and their families.
- Strong organisational and computer literacy skills
- Genuine motivation to work in the Disability Support sector
