**About the Role**:
We are seeking a dedicated and experienced Support Coordinator to join our Sydney-based team. The Support Coordinator will work closely with NDIS participants, families, and service providers to help clients understand their NDIS plans, connect with suitable supports, and build their capacity for independence.

**Key Responsibilities**:

- Conduct needs assessments in line with NDIS plans.
- Develop, implement, and monitor person-centred support plans.
- Coordinate and liaise with participants, families, and multiple service providers.
- Ensure participants access the right mix of funded and mainstream supports.
- Maintain accurate documentation and case notes in accordance with NDIS guidelines.
- Advocate for participant rights and promote community participation.
- Review participant progress and adjust plans as required.

**Skills and Qualifications**:

- Diploma or higher qualification in Community Services, Disability, Social Work, or related discipline.
- Minimum 2 years of experience in support coordination, case management, or a similar role within the disability or community sector.
- Strong understanding of the NDIS framework and person-centred practice.
- Excellent interpersonal, communication, and problem-solving skills.
- Must have NDIS Worker Screening Check
- Must have Working with Children Check
- Current First Aid and CPR (preferred).
- Must have current driver’s licence and access to a reliable vehicle.
