**About the role**
This position is responsible for delivering Support Coordination for a case load of National Disability Insurance Scheme (NDIS) participants.
On a daily basis you will instil best practice approaches, manage and undertake a Support Coordination case load of NDIS funded participants (or their representative) and relevant service providers to assist plans coming to fruition.
- Support and educate individuals and/or their representatives on their rights and responsibilities concerning their funded plans
- Engage, build relationships and negotiate with the public sector and community based service systems for delivery of appropriate supports in accordance with NDIS plans
- Manage appropriate NDIS funded referral arrangements to and from Community Care and develop Service Agreements
- Support and educate individuals and/or their representatives on their rights and responsibilities concerning their funded plans
**About You**
**Qualifications**
- Minimum Diploma in Disability, Community Services or relevant.
- Driver’s license.
- First Aid Certificate.
**Experience**
- Minimum 1 year experience delivering Support Coordination in NDIS
- Minimum 3 years’ experience working in Community Services using the person-centred approach
- Working with people with mental health issues and those form a culturally and linguistically diverse background to meet their goals
**Skills**
- Self-motivated with the ability to work independently, and as part of a team
- High level adaptable communication and negotiation skills
- High attention to detail and time management
- Maintaining required information within set time frames including processes to ensure accuracy of data
**Desirable**
- Mental Health First Aid Certificate

We are offering an attractive salary from $78,860.08 p.a. pro-rata plus Super.
