Due to our continued growth, we are currently seeking an enthusiastic and experienced Care Coordinator to join our team and help us deliver high-quality support services to our valued participants.

**Role Overview**

The Care Coordinator will play a key role in overseeing support services for a portfolio of NDIS participants. You will coordinate daily supports, manage staff rostering, maintain care plans, and ensure services align with each participant’s NDIS goals.

**Key Responsibilities**
- Coordinate and monitor supports for participants in line with their care plans
- Develop, review and update person-centred support plans
- Communicate with participants, families, support staff and external providers
- Manage shift rosters and staff allocation
- Maintain accurate case notes and documentation
- Ensure compliance with NDIS and organisational policies
- Respond to incidents and escalate concerns appropriately

**About You**
- Previous experience in a Administration/Care Coordinator or similar role in disability or community services
- Strong understanding of the NDIS and person-centred care principles
- Excellent communication and organisational skills
- Ability to work independently and as part of a team
- A genuine passion for supporting people with disabilities

**Required Qualifications & Checks**
- Certificate IV or higher in Disability, Community Services, or a related field
- NDIS Worker Screening Check
- National Police Check (current)
- First Aid & CPR Certificate
- Valid Driver’s Licence

Pay: $34.00 - $37.00 per hour

Expected hours: 15 - 20 per week
