**About the opportunity**:
The role of the Care Coordinator is to support NDIS participants to develop and coordinate a range of services that support and promote their attainable goals. By employing professional skill and knowledge, the Care Coordinator will assist participants to remain living at home through the promotion of independence and choice.

High-level duties include managing daily tasks within the Direct Supports Team (work with Operations Manager and other Care Coordinators and Allied Health Team), ensuring the participant's wellbeing and being a point of contact for the participant’s support requirements, and maintaining and collaborating connection between the support team and participants.

**Responsibilities include**:

- Intake of new Participants and carrying out the participant induction procedure by educating Participants on NDIS and its processes
- Complete and maintain onboarding documents of participants
- Carrying out Risk Assessments
- Prepare participant’s Support Plans and PCP’s
- For High Care Needs Participants - Ensure medication plans and Peg feed plans and appropriately trained rostered staff are in place.
- To be the main point of contact for general participant inquiries from both internal and external sources, referring queries were necessary to the appropriate member of the team.
- Support Participants in decision-making and coordinate a program for older people or people with a disability in the community and understand consumer-directed care.
- To be part of the Business team and take responsibility for the day-to-day administration
- Managing the reports and ensuring all reports are completed on time and are kept up to date
- Perform any administrative tasks as required by the team

**Desirable / Requirement**:

- Appropriate certification which is relevant to the disability industry (NDIS Screening Check, Certificate III, IV in Disability, Certificate IV in Individual Support)
- A valid overseas or Australian driver's License
- Insured car
- National Police Check
- WWCC optional
- COVID Vaccination Certification
- Available to start asap

**Knowledge & Experience**:

- Previous experience within a disability or age care environment.
- Well-developed communication (written, verbal and listening) and interpersonal skills to empathetically approach and manage the sensitivities of NDIS participants.
- Computer literacy in word processing, client management systems, spreadsheets, and calendar management.
- Effective time management and organisational skills to efficiently plan and prioritise work tasks.
- The ability to work autonomously with a team focus to contribute to our team and our core values.

**Job Types**: Full-time, Casual

**Salary**: $60,000.00 - $64,999.00 per year
