**The Company**
Relieve Disability Service Pty Ltd is a registered NDIS provider headquartered in Werribee, Victoria, dedicated to delivering flexible, person-centred supports that help people with disability and older Australians live safer, fuller and more connected lives. The organisation offers a broad service mix—including support coordination, community participation, home-care assistance, transport, short-term accommodation, community nursing and tailored home modifications such as wheelchair ramps and accessible bathrooms—designed to translate individual goals into practical outcomes. Guided by values of integrity and innovation, Relieve partners closely with participants, families and allied-health professionals to remove barriers, champion inclusion and ensure every client’s funding achieves maximum impact for independence.
A Community Lead Worker at Relieve Disability Service designs and delivers capacity-building programs that connect Relieve participants with their communities. The role conducts holistic needs assessments, drafts individual participation plans, and coordinates multidisciplinary supports across nursing, therapy and daily living services. It leads outreach events, supervises disability support staff, and monitors service quality through reviews and client feedback.
