Position Summary.

As a Specialist Support Coordinator, you will work closely with NDIS participants who have complex needs, helping them to navigate challenges and implement their plans effectively. This role involves a higher level of coordination, often where barriers to service access or engagement require intensive, time-limited support to ensure long-term stability.

Key Responsibilities:

Provide Specialist Support Coordination for participants with high-level, complex needs (e.g., mental health, dual diagnosis, housing instability).

  • Work collaboratively with participants, families, carers, and service providers to develop and implement action plans aligned with participants' NDIS goals.
  • Build capacity in participants and their support networks to manage and coordinate their supports independently over time.
  • Resolve crises and barriers impacting access to supports, including liaising with clinical and mainstream services.
  • Maintain up-to-date documentation, case notes, progress reports, and data entry into case management systems.
  • Attend regular supervision, team meetings, and professional development activities.
  • Ensure all services delivered comply with NDIS Practice Standards and the organisation's policies and procedure

Essential:

Qualification in Social Work, Psychology, Allied Health, Community Services, or relevant field.

  • Minimum 2 years' experience working with people with complex needs in disability, mental health, or related sectors.
  • In-depth knowledge of the NDIS and Specialist Support Coordination function.
  • Strong problem-solving, crisis intervention, and conflict resolution skills.
  • High level of written and verbal communication skills.
  • Demonstrated ability to work independently and manage competing priorities.
  • Current Working with Children Check, National Police Check, and NDIS Worker Screening.
  • Valid Driver's Licence and access to a reliable vehicle (if outreach/home visits are required).
