**How you’ll make an impact**

The Team Leader for CareChoice Homes is a fantastic opportunity for an experienced leader to join CareChoice and influence the continued growth of the business.

Reporting to the Manager of Client Engagement and Business Development, the role of Team Leader, CareChoice Homes is integral to the effective delivery
of services in CareChoice Homes. The role is responsible for the delivery of high quality SDA service provision, collaborating with participants, their families,
carers, Support Coordinators and broader care teams. The role is responsible
for supporting the entire participant journey from enquiry to tenancy.

A day in the life as a CareChoice Homes Team Leader, may see you supporting our Participant with:

- Coordinating the pipeline of eligible participants for placement in CareChoice
Homes.
- Providing assistance and advice to convert appropriate enquiries into residents of CareChoice Homes.
- Ensuring all homes are tenanted within 60 days of enrolment and vacancies
are strictly monitored and filled.
- Involvement in key stakeholder relationships and SDA partnerships.
- Providing in-kind Coordination of Support to clients, to enable funding (for
eligible clients) required for residency in a CareChoice Home.
- Documenting processes and engaging in continuous improvement activities,
ensuring that all aspects of SIL allocations occur in line with NDIS practice
standards and CareChoice values and strategy.
- Complete a thorough matching process for all participants ensuring all aspects of individual participant preference and co-tenancy are taken into consideration.
- Monitor progress of SIL submissions and SDA funding approval in collaboration with Funding Manager (finance).
- Collating all appropriate information for presentation to the CareChoice
Executive panel for approval of eligible potential residents of a CareChoice Home
- Meet or exceed KPIs set in the performance appraisal framework, which is
reviewed annually.
- Participate in review of the CareChoice database, identifying participants
who are potentially eligible residents for CareChoice home.
- Provision of in-kind Support Coordination hours necessary to ‘fast-track’
identified participants towards successful NDIA decision making.
- Perform appropriate matching with the aim of long-term residency.
- Support CEMs to maintain regular contact with key stakeholders on
allocated programs and review programs as specified in communication
plans/ support plans an
