We are seeking an experienced and dedicated Care Manager with AHPRA registration to join our team. The Care Manager will play a pivotal role in ensuring the provision of exceptional care services to our clients and will provide strong leadership to Clinical and Care Coordination team.

**Key Responsibilities**:

- Manage a team of coordinators and nurses, providing leadership, guidance, and support to ensure efficient service delivery.
- Oversee caregiver training and development programs to ensure our team delivers the highest level of care.
- Conduct clinical assessments to determine clients' care needs and create personalized care plans.
- Write and review care plans, ensuring they are aligned with client requirements and regulatory standards.
- Collaborate with clients, families, and healthcare professionals to maintain open communication and address any care-related concerns.
- Manage incident reporting and continuous improvement.

**Qualifications**:

- AHPRA registration as a registered nurse (RN) or other relevant healthcare qualifications.
- Proven experience in the healthcare industry, particularly in NDIS and Home Care Packages.
- Strong leadership skills with the ability to manage and motivate a team.
- Excellent communication and interpersonal skills.
- A deep commitment to providing compassionate and client-centred care.
- Understanding of relevant regulations and standards in the healthcare industry

Pay: $86,179.86 - $124,751.60 per year
