**Our story**

In 2020, Calvary and Medibank formed a joint venture to deliver a virtual hospital, an innovative, integrated virtual hospital that leverages a virtual care center (VCC) to deliver hospital-level care to South Australians in the comfort of their homes.

The Calvary and Medibank Joint Venture** **(CMJV)** **has successfully delivered care to over 180,000 patients across NSW, VIC, QLD, SA and WA. Care is delivered through a digital platform with partners and a strong multidisciplinary workforce strategy at its core.

CMJV delivers the My Home Hospital SA Health program, it has delivered care to more than 6,500 patients since its launch in January 2021.

**The Opportunity**:
As a Care Coordinator, you will utilise your clinical, people and communication skills and will be responsible for managing acute patient care, enhancing and promoting their quality of life. This is a busy service that demands the best talent to help us lead and deliver this great initiative. Our team work across a 7 day roster between the hours of 7am and 10pm, we offer both full time and part time opportunities. There is also a on-call component between the hours of 10pm and 7am where you will be responsible to manage any incoming calls from patients, referrals etc. This positon is Adelaide based working in our Virtual Care Centre (VCC) in Wakefield Street CBD. We offer flexible working options for employees wanting the ability to work from our office as well your home.

**You will add value by**:

- Managing patient’s acute care health requirements in a professional and safe manner
- Establishing, monitoring and coordinating patients' treatment plans, utilising virtual technology & in partnership with home visiting provider team
- Educating patients about their condition and connect patients with their health care providers
- Identify/escalate to your direct line manager and actively manage clinical risks
- Manage compliance in line relevant discipline and industry codes, competencies, standards, and acts
- Maintain staff and consumer confidentiality according to professional standards
- Record and process clinical and administrative information in an accurate and timely manner
- Develop and maintains professional competence by self-education
- Support clinical team and acts as role model

**About you?**

As an outstanding communicator, your acute assessment skills, care planning, coordination skills and intuitive people skills will be central to your success. You will be naturally empathetic, observant, attentive, trustworthy, and committed to making your patient feel safe and comfortable. You will be knowledgeable about acute health care practice’s and are surrounded by likeminded team members who live and breathe a service centric culture by providing an impeccable experience to consumers and stakeholders together with:

- Minimum 3 years' experience as a Registered Nurse with an unrestricted AHPRA registration
- Strong clinical reasoning & clinical assessment skills
- Knowledge and experience (preferred) in:

- Acute care management in hospital/community/nursing
- Assessment & management of deteriorating patient
- Multidisciplinary teamwork
- Ability to work in a fast-paced busy environment
- Ability to work all shifts across 7 days a week
- Proficient computer skills to enable fast and accurate data entry into patient electronic medical records.
- Sound logístical and coordination capability and experience
