**The role**: To support individuals living with advanced dementia or experiencing social isolation by delivering person-centred, meaningful engagement. This includes providing social, emotional, and physical support through tailored activities, exercise, and outdoor experiences that enrich quality of life and promote wellbeing
**Shifts are Monday to Sunday**: 10:45am - 5:15pm
**What you'll do**:

- Provide one-on-one support to residents living with dementia, experiencing cognitive decline and behavioural disturbances or are socially isolated
- Liaise with Careworkers, Clinical Staff, Allied Health and Social Team regarding individual customer engagement preferences, emotional support strategies, health status and care plan
- Influence and inform the Changed Behaviour support strategies and recommendations through the appropriate clinical staff

**About you**:
You are a talented and passionate Care Worker proven experience in supporting people living with Dementia. You want to make the most of your expertise, do what you do best and thrive on the challenge of providing our customers with quality care.
- Excellent communication and problem-solving abilities
- A passion for supporting choice and independence
- High level customer service experience
- Share the ACH Group culture and values

**You will need**:

- To care about supporting older people to live a Good Life.
- Certificate III in Individual Support Aged Care or a nursing student that has undertaken a year of their course including their placement
- A satisfactory Police Check for working with vulnerable groups less than 12 months at commencement
