The Family Support Worker is responsible for looking after the participants and helping them build their
independence and assist them in building their capacity.

Bay One Care Services seeks a client-focused Family Support Worker to join our growing team.

We understand the challenges of at-home care; as such, the pay rate we offer is $33.41 - $44.20 with

11.5% superannuation on top. We also cover the cost with the km travelled between shifts and any

transportation on shift.

We’re looking for those special few who genuinely want to engage with and contribute to our client’s

independence and well-being.

**MAIN DUTIES/RESPONSIBILITIES**
- Evaluate clients' needs and design, develop, and implement educational, training, and support programs.
- Conduct client interviews to assess the nature and severity of their challenges.
- Track and report on the progress and outcomes of clients.
- Refer clients to appropriate agencies for additional support and services.
- Assess community needs and available resources in areas such as health, welfare, housing, employment, training, and other essential services.
- Collaborate with community groups, welfare organizations, government bodies, and private sector entities to address community issues and raise awareness of available resources and services.
- Provide support to families, offering education and care for children and individuals with disabilities in adult service units, group housing, and government institutions.
- Supervise offenders on probation or parole, ensuring compliance and facilitating their rehabilitation.
- Help young people overcome social, emotional, and financial challenges.
- Prepare reports and proposals for funding, resources, and submissions to government agencies and other organizations.

**QUALIFICATION**
- First Aid/CPR
- Police Check
- Working with Children Check (Blue Card)
- NDIS Worker’s Screening Check (Yellow Card)
- NDIS Workers’ Orientation Module Certificate
- NDIS New Worker NDIS Induction Module

Certificate
- NDIS Supporting Safe and Enjoyable Meals

Module Certificate
- NDIS Supporting Effective Communication

Module Certificate
- Diploma in any health-related qualification.

**Job Types**: Full-time, Permanent

Pay: $75,000.00 - $80,000.00 per year

**Experience**:
