Care Coordinator

The role of Care Coordinator is to support clients to remain living at home through the promotion of independence and choice. The Care Coordinator will ensure clients receive quality care by employing professional skills and knowledge to supervise support staff, problem solve and oversee the day to day running of the participants home.

The Care Coordinator is to be responsible for maintaining a team environment that fosters positive relationships between all parties involved and alleviate the stress experienced by clients, cares and family.

About you.
- Qualification in Human Services - minimum Diploma level
- Substantial experience working with people with disabilities including mental health in a home or community setting
- Strong knowledge of the Nation Disability Insurance Scheme
- Ability to communicate with clients and their families to problem solve and gain feedback
- Development and coordination of Individual Care Plans
- A strong understanding of effective service delivery and experience in building support worker teams to achieve best outcomes for our participants
- Monitoring required documentation completed appropriately
- Demonstrated ability to supervise, lead and support staff in their role
- To assist with staff training and development
- Sound knowledge of WHS and client risk assessments
- Ability to maintain professionalism, integrity and confidentiality
- Excellent oral, computer and written communication skills including report writing and data entry
- A current driver’s license and ability to visit client homes and work offsite

**Job Type**: Casual

**Salary**: $28.96 - $36.20 per hour

Schedule:

- Afternoon shift
- Day shift
- Evening shift
- Flexible hours

Supplemental pay types:

- Penalty rates
