**Full time Support Coordinator, Belfield Office**

Affective Care are looking for people who are committed and passionate about improving the lives of people living with disability.

In this role you will liase with NDIS participants, their families or respresentatives to support participants to achieve their goals. Some of your duties will include
- Preparing and attend for plan review meetings with the Participant, Family or Representative.
- Maintaining an Up-To-Date knowledge of the NDIS Systems and Practices;
- Engaging with NDIA Staff and LAC’s;
To be successful for this role you will have:

- Experience providing NDIS Support Coordination Services.
- Excellent Time Management and Administration with the ability to maintain Accurate, Professionally appropriate and Current Records and Written Reports.
- A well rounded understanding of the NDIS.
- Demonstrated experience with Facilitating Multi-Disciplinary Coordinated Planning and Develop Collaborative Relationships and Networks between Service Providers.

**Qualifications & Experience**:

- Preferably 1 -2 years of Minimum Experience in this role or similar
- Experience with Engaging with Stakeholders and Ability to Adapt
- Preferably Qualifications with in the Community Services, Mental Health and Disability/and or Similar.
- NDIS Workers Screening Check - Required

- First Aid Certification (preferred)
- NDIS Screening Check (preferred)
- Full Driver Licence and car (required)
