As a **Care Partner**, you’ll guide customers through their care journey with compassion. Working closely with families, clinical teams, and external providers, you’ll develop holistic health plans, coordinate services, and ensure customers receive safe, person-centred care that enables them to age well at home or in a place of their choosing.
**This is a hybrid opportunity with the option to work 2 days from homeand 3 days from your closest Australian Unity Branch Office.**
**Key Accountabilities**
- Build trusted relationships with customers and support them to achieve their goals at home.
- Conduct onboarding assessments, annual reviews, develop holistic health plans, and coordinate multidisciplinary services.
- Collaborate with clinical teams, families, and external providers to deliver safe, person-centred care and optimal outcomes.
- Support customers through care transitions, including hospital discharge or changes in health needs, and follow up with offboarding where needed.
- Maintain accurate records and reporting in enterprise systems, participate in audits, and contribute to quality and continuous improvement initiatives.
- Identify opportunities for additional services and support to enhance customer outcomes and contribute to service growth.
- Promote safety, quality, compliance, and a culture of continuous improvement across all activities.
**What You’ll Bring**
- Experience in community or aged care, including case management and coordinating multidisciplinary care in the home.
- Strong customer focus, with the ability to build trusted relationships and handle sensitive conversations with empathy.
- Excellent verbal and written communication skills & tech savvy with Microsoft Suite and CRM systems
- Knowledge of funding streams, quality standards, audits, and experience supporting care transitions such as discharge planning.
- Strong decision-making, problem-solving, time management, and adaptability in a fast-paced environment.
- Collaborative mindset, able to work effectively in a team-based, matrix environment, liaising with families, clinicians, and external providers.
- Relevant qualifications (Certificate IV or Diploma in Community or Aged Care desirable).
