From assessing needs to coordinating services, you’ll make sure every client receives the right support, tailored to their goals and funded care options.In the role you will:* Support seniors with **care planning, assessments, and coordination** to help them live independently.* Conduct assessments and create initial care plans and budgets in collaboration with clients* Build strong relationships with clients and families, tailoring services to individual needs.* Advocate for clients as their circumstances change and ensure they receive the right support.**What you’ll bring*** Experience in case management in aged care, or community services (Diploma or equivalent preferred).* Understanding of **Home Care Packages** and confidence with CRMs/digital tools.* Experience conducting assessments in home settings and delivering exceptional customer service.* A caring nature, strong communication skills, and ability to build trust quickly.* Current driver’s licence.We provide meaningful opportunities and choice – for the individuals and communities we support, and for the dedicated people who make it happen.
