Clinical Care Partner / Registered Nurse - Aged Care

Impact you will have in this role

As a Clinical Care Partner, you will play a pivotal role in shaping how older people experience care in their own homes. Your clinical insight will help ensure that every person receives safe, tailored and dignified support that enhances independence and wellbeing.

You will provide guidance and specialist clinical consultation, working collaboratively with Care Partners to support complex or high-risk participants. You will ensure that clinical best practice is embedded in every care plan and that participants are empowered to live well, even with significant health challenges.

By bringing a clinical lens to Eremea's holistic, person-centred approach, you will strengthen the team's capacity to respond compassionately and effectively to changing needs, reduce avoidable hospitalisations, and promote comfort and dignity across all stages of ageing.

This is a deeply purposeful role where your nursing expertise will not only influence individual outcomes but also uplift the standard of care across the Support at Home Program. 

Key Responsibilities

  • Provide clinical advice, review care plans, and guidance in managing complex or high-risk participants
  • Ensure clinical governance principles underpin all care decisions, documentation and outcomes
  • Lead and contribute to clinical assessments, risk reviews and advanced care plans for participants with chronic or end-of-life needs
  • Support multidisciplinary collaboration with GPs, allied health professionals, support workers, and families
  • Work alongside Care Partners to ensure timely escalation and management of clinical concerns such as wound care, mobility issues or medication safety
  • Monitor outcomes and ensure interventions are reviewed and adjusted in response to changing needs
  • Support Care Partners through clinical education, reflective practice and quality reviews aligned with the Strengthened Aged Care Quality Standards
  • Maintain high standards of documentation and ensure all clinical activity complies with the Support at Home Program Manual and relevant quality frameworks

About You

You are an empathetic clinician who thrives on collaboration and values relationship-based care. You balance clinical rigour with compassion and are committed to improving outcomes for older Australians. You will also have:

  • Qualification as an Enrolled Nurse or Registered Nurse, with current AHPRA registration
  • Minimum of 3 years experience in community, aged care or home-based clinical care
  • Strong understanding of aged care reforms, clinical governance and risk management
  • Experience in assessment, case management, and person-centred care planning
  • Excellent communication skills with the ability to build and maintain strong stakeholder relationships both internally and externally
  • Flexibility to work in an evolving environment, with the ability to pivot and manage multiple priorities in a collaborative environment
  • Current Police Check or NDIS Worker Screening Check, and full COVID-19 vaccination
