**Role Overview**:
This is an exciting opportunity to join UnitingSA’s team as the Case Manager. The Case Manager will provide intensive case management support to people experiencing homelessness or at risk of homelessness. The Case Manager will drive responsive and client focused services by encouraging active client participation and individual learning outcomes, engaging the client with the larger community and other relevant agencies. The Case Manager will also be responsible for the provision of services consistent with the KPI’s outlines in the Western Adelaide Homelessness Service agreement.
**Key Responsibilities**
- Provide case management services to people experiencing homelessness or at risk of homelessness in line with UnitingSA case management framework.
- Provide high quality, goal-orientated services to meet the needs of clients and external customers.
- Manage referrals, assessing needs and providing appropriate services
- Respond to client enquiries and complaints.
- Provide linkages and referrals to other agencies (where appropriate)
- Identify and set priorities, and manage high workloads to meet deadlines.
- Deliver engaging and relevant activities and programs that deliver relevant service outcomes.
- Provide information and referral pathways to support and encourage self-management
**Skills & Experience**:
- Relevant tertiary qualification in Human Services or equivalent (Degree in Social Work, Education, Youth Work or equivalent experience) essential.
- Qualification or training specifically related to working with individuals and families with multiple and complex needs within the Homelessness sector (desirable).
- Significant experience in working in a human services setting and knowledge of a range of appropriate models of intervention.
- Experience in preparation of reports.
- Experience in working with marginalised people, including skills in case planning and complex case management.
- Experience and knowledge in Case Management including assessment, planning, referral and advocacy.
- Deliver a service that acknowledges diversity and the impact of multiple and complex needs of people experiencing homelessness or at risk of homelessness.
- Work in partnership with people experiencing homelessness or at risk of homelessness from a strengths-based perspective.
- Co-ordinate, develop and maintain links with other government and non-government agencies.
- Maintain a database and work with the leadership group to analyse data for the purposes of reporting and improving upon service delivery.
- Report trends and issues arising for people experiencing homelessness or at risk of homelessness.
