The Role
As the Case Manager/Referral Worker we are looking for someone that has a passionate commitment to working with people with AOD, Mental Health and homelessness issues. You will provide case management support to vulnerable and marginalised clients with multiple co-occuring needs. Working with individuals in crisis or those that are identified as frequent presenters to the service to develop and implement case plans that promote the best interests of the client, including organising ’wrap around’ services for the client when required and appropriate. The role also involves engaging with first time presenters to allow for early intervention.
You will Successfully
- Have a comprehensive concept of harm minimisation and will help clients make positive transformations in their lives.
- Your role includes assessing client needs from intake to exit, developing / implementing individual case plans, and providing advocacy and referrals to external service providers.
- The position is for a solo worker (job share) that is part of and works within a close-knit team of Support Workers within the Sobering Up Unit.
The part time, permanent position located in Adelaide, SA reports to the Sobering Up Unit Team Leader.
You Will Have
- Have a Tertiary Health Qualification / Diploma in Community Health Services Discipline or related field with significant practical experience in a social service environment
- Be able to demonstrate your ability to work within the parameters of a case management model
- Know how to identify risk factors (for yourself and our clients) and take appropriate action including referrals
- Have successfully worked with individuals from different cultures and backgrounds with complex needs
- Attend relevant meetings that are responsive to this client group e.g. Coordinated Care Meetings, Vulnerable Persons Framework meetings, By Name List Meetings etc
- Be able to work safely and effectively autonomously and as part of a team
