Services are free, voluntary, accredited and delivered by professional and committed staff.We are currently seeking a value driven and proactive Case Manager to provide, as part of a team, case management support and mobile outreach support to people, in a community based and / or residential based setting. The support is underpinned by a practice framework, using resilience, trauma informed and strength-based practices, to support people to build their own capacity and resources on a journey of transformation.

The service is a 24/7 service which provides immediate temporary supportive accommodation, on site and within the community.

This is a full-time position and is based in Ashmore City.

You will successfully
- Establish relationships with individuals that access the service that encourages self-determination and participation
- Provide case management support to individuals in accordance with TSA case management models and standards of practice
- Individual case planning in collaboration with the individual including a range of interventions that address the needs or goals of the individual, as well as barriers to achieving those
- Provide ongoing assessment of the individuals’ presenting issues, vulnerabilities and needs from initial intake to exit
- Assist individuals to access support services and establish community supports, and where needed provide advocacy
- Regularly review progress and outcomes with the person of individual case plans and where needed taking action

You will have
- Tertiary qualification in Community Services, Social Work, or related field. A degree-level qualification is preferred with a minimum diploma level
- Relevant experience in a social service environment including in client assessment and case management (e.g. homelessness)
- A valid Employee Working with Children Check (Blue Card)
- A First Aid Certificate and CPR
- A QLD Driver's licence
- A willingness to undergo National Police Check
