**Role Overview**: We are seeking a compassionate and proactive NDIS Support Coordinator to assist our clients in navigating the National Disability Insurance Scheme (NDIS). In this role, you will work closely with participants to develop, implement, and monitor their NDIS plans, ensuring they receive the support and services needed to achieve their goals.

**Key Responsibilities**:

- **Client Support**: Work with NDIS participants to understand their needs, goals, and preferences, and help them navigate the NDIS system effectively.
- **NDIS Plan Management**: Assist clients in developing their NDIS plans, including setting goals, identifying appropriate supports, and coordinating service delivery.
- **Service Coordination**: Liaise with service providers, community resources, and stakeholders to ensure a seamless delivery of services.
- **Advocacy**: Advocate for participants’ needs and rights, ensuring they receive fair and equitable access to NDIS services.
- **Monitoring and Review**: Regularly review and update clients' NDIS plans and progress, making adjustments as needed to meet changing needs.
- **Documentation**: Maintain accurate and up-to-date records of client interactions, plans, and service delivery.

**Key Requirements**:

- **Experience**: Previous experience in support coordination, disability support, case management, or a related field is preferred.
- **Knowledge**: Strong understanding of the NDIS framework, policies, and procedures.
- **Skills**: Excellent communication, interpersonal, and organizational skills.
- **Empathy**: A genuine passion for working with individuals with disabilities and a commitment to improving their quality of life.
- **Qualifications**: Relevant qualifications in social work, community services, or a related discipline are advantageous.
- **Technology**: Proficiency with MS Office Suite

**Job Types**: Part-time, Permanent

Pay: $60,240.50 - $101,147.31 per year

Expected hours: 24 per week
