The Practice Lead plays a critical role in operationalising Yooralla’s practice leadership, supported decision making and active support programs. Reporting to the Program Director, Practice, they will provide practical input on the design and implementation of the programs based on their experience as a skilled Service Manager and/or Disability Support Worker.

**Key Responsibilities**:
**Specific Role Responsibilities**
- Drive implementation of Yooralla’s practice leadership, supported decision making and active support programs by working directly with Operational Managers to embed the programs in services.
- Work in collaboration with the Program Director, Practice to undertake design, implementation, assessment and review of strategies and initiatives arising from the programs.
- Provide coaching and mentoring to Service Managers on practice leadership to drive service excellence in their services.
- Provide input and advice into the design of new policies, procedures and any associated materials relating to practice leadership, active support and supported decision making to ensure that they are fit for purpose in the Yooralla operating context. Consult with relevant stakeholders in the development of applicable documentation.
- Work with the Customer Rights and Empowerment Team to support activities and programs to embed supported decision making.
- Support the collection of any data to monitor program impact and report on activities and outcomes; provide improvement recommendations based on measurement findings.
- Act as a Yooralla role model and ambassador for the Program and its role in delivering best practice evidence based disability support.
- Partner with staff, clients and families to ensure that the Program is responsive to what is important to them.
- Work closely with the Program Director, Practice to execute initiatives that underpin the ‘Practice’ segment of the Rights, Quality and Safety Framework.
- Actively participate in communities of practice and attend relevant sector events to remain up to date with emerging and contemporary evidence-based best practice.
- Escalate service risks to support prompt and effective remediation, in partnership with Operations.
- Other related duties and tasks as directed.

- Promoting and protecting the human rights of people with disability, in accordance with the UN Convention on the Rights of Persons with Disabilities (CRPD) (2008).
- Working to ensure high-quality and safe supports and services for NDIS participants,
meeting and exceeding the NDIS Practice Standards and
Quality Indicators.
- Meeting obligations under all relevant Victorian and Commonwealth legislation.
- Working in accordance with the Yooralla Code of Conduct and Employee Manual.

**Selection Criteria**:
**Mandatory Requirements**
- NDIS Worker Screening Check.
- Current driver's licence to drive in Australia.
- Working with Children Check.
- Experienced disability support worker who can demonstrate service provision in accordance with the principles of active support.

**Highly desirable**
- Lived experience of disability.
- A post graduate qualification in a relevant field.
- A qualification in a relevant field or equivalent such as disability, community services or health.

**Professional Experience**
- Specialist knowledge and understanding of issues affecting people with disability.
- Extensive experience in frontline disability support provision with clients with a wide range of disabilities, including those with complex support needs and behaviours of concern.
- Demonstrated commitment to upholding a person’s human rights, independence and dignity.
- Demonstrated experience in promoting supported decision making and elevating the client’s voice.
- Understanding of human rights informed quality management principles and standards.
- Implementation, adherence to and promotion of policies and practice standards in relation to the provision of disability support services.
