**About the role**
As a Care Management Officer in the City’s Community Care department, you will be responsible for providing services in line with the Home and Community Care Program for Younger Persons (HACC-PYP). You will have a primary focus of working with NDIS clients and supporting younger people with disabilities to obtain appropriate supports and develop individual support plans.
Key responsibilities include:
- Develop and maintain effective referral, assessment and review mechanisms and care plans to manage the needs of prospective and existing service recipients.
- Implement care management practices which promote privacy and confidentiality.
- Ensure appropriate planning, provision, co-ordination, integration, development and evaluation of Care Management Services.
- Respond to new clients and prioritise for a range of Community Care Services
- Provide and maintain up-to-date knowledge of developments in Care Management Practices, management of people with disabilities and their carers.
**About you**
To be successful in this role, you will be experienced in the preparation and review of Care Plans, as well as knowledge of issues relating to people with disabilities. You will have the ability to liaise with a diverse range of people, have a professional and empathetic approach to your work and enjoy dealing with complex/challenging situations.
Essential Experience:
- Awareness and support of the philosophies of consumer directed care, promotion of independence and non-intrusive care and general disabilities, including a collaborative approach to decision making.
- Extensive knowledge of governmental and community resources available for people with disabilities and their carers.
- Comprehensive knowledge of holistic assessment and care management practices.
- Sound knowledge of the National Disability Insurance Scheme (NDIS) and the Home and Community Care Program for Younger Persons (HACC-PYP).
- Knowledge of comprehensive assessment and review practices and the preparation of Care Plans and service delivery plans.
- A minimum of Diploma in Community Services or other tertiary qualification in Human Service field or relevant experience.
