**Position Overview**:
As an NDIS Support Coordinator, you will be at the forefront of empowering individuals with disabilities to achieve their goals and improve their overall quality of life. Your role will involve guiding clients through the NDIS process, collaborating with various stakeholders, and crafting personalized plans tailored to each individual's unique needs.
**Key Responsibilities**:
- **Client Engagement**: Cultivate and maintain positive relationships with clients and their families, gaining a profound understanding of their aspirations, challenges, and preferences.
- **NDIS Plan Management**: Navigate the intricacies of NDIS plans, ensuring the efficient allocation of resources and adherence to guidelines.
- **Collaboration**: Collaborate closely with service providers, healthcare professionals, and community organizations to establish a comprehensive support network for clients.
- **Advocacy**: Serve as a staunch advocate for clients, ensuring their voices are heard and their rights upheld within the NDIS framework.
- **Documentation**: Maintain meticulous and up-to-date records of client interactions, progress, and plan adjustments in accordance with regulatory standards.
**Qualifications and Skills**:
- Cert IV in Mental Health / Disability or preferably a Diploma in case management
- Demonstrated expertise in the NDIS framework and processes.
- Strong interpersonal and communication skills.
- Ability to work collaboratively with a diverse range of stakeholders.
- Empathy, compassion, and a dedicated commitment to promoting independence and inclusion.
