**About Us**:
Established in 1982, Alzheimer’s WA provides care, support, awareness and information to assist people living with dementia, as well as their families and carers.
- Our services include advocacy, education, partnerships, training, consultancy and holistic person-centred care programs, based on a philosophy of understanding the lived experiences of those living with dementia. We provide CHSP, HCP home care packages, day and overnight respite in our three homes based in Perth, Mandurah and Albany.- **Position Summary**:
**Responsibilities**:
Duties of this position include, but are not limited to:
- Onboarding new NDIS clients and developing Service Agreements to reflect the supports, funding and services requested.
- Coordination and development of supports to a caseload of NDIS participant living in the community.
- Conducting risk assessments and home visits, developing support plans for the participants and setting up those supports and services accordingly.
- Certifying new participants have updated medication profile and signing sheets.
- Implementing and monitoring behaviour support plans and organising ongoing training with support workers.
- Working closely with participants and the support workers to ensure quality service delivery.
- Monitoring restrictive practices, incident reports and reviewing progress notes.
- Conducting assessment and regular reviews with focus on personalised support for the participants in line with NDIS standards.
- Liaising with schedulers for provision of appropriate and timely client services.
- Maintaining clients records accurately and confidentially.
- Actively participating in continuous improvement in delivery of services and processes and supporting participants during crisis.
**Essential Criteria**:
- Diploma / Degree in community service, with a minimum of 2 years proven experience in disability (NDIS).
- Previous experience as a NDIS Service Coordinator or similar
- Demonstrated organisation skills, including the capacity to successfully manage competing priorities, meet deadlines and maintain attention to detail
- Full working rights
- First Aid and CPR certificate
- A valid WA ‘C’ class driver’s licence
- NDIS Worker Screening
- Up-to-date COVID Vaccination
**Desirable**:
- Knowledge of the broad range of services and referral pathways for people with dementia and their carers.
