**About Us**:
Established in 1982, Alzheimer’s WA provides care, support, awareness, and information to assist people living with dementia, as well as their families and carers.
Our services include advocacy, education, partnerships, training, consultancy and holistic person-centred care programs, based on a philosophy of understanding the lived experiences of those living with dementia. We provide CHSP, HCP and NDIS packages and day/overnight respite in our four homes based in Perth, Mandurah and Albany.
**Position Summary**:
The role reports to the NDIS Manager and focuses on coordinating NDIS Services for the participants to help them achieve their goals. The support coordinator will support the participants to achieve their goals by helping them to understand and use their NDIS funded supports and connect with providers. The Support coordinator will support participants to build their confidence and skills to manage their supports independently, giving them choice and control over their supports.
**Responsibilities**:
Duties of this position include, but are not limited to:
Support the participant to develop a greater understanding and confidence in their NDIS plan, service agreements and develop a plan budget for the services they choose.
- Connect participants with NDIS providers, community, mainstream, and other support services.
- Support participants to build confidence and skills to use and coordinate their services.
- Explore options and assist participants to make informed decisions
- Support participants to resolve issues that may arise with their services.
- Support participants to navigate NDIS to implement their plan and ongoing review
- Assist participants to prepare for their NDIS plan review meetings
- Provide advice and support throughout participants NDIS journey
- Advocate on behalf of participants and address any provider issues.
- Support participants to ensure a mix of supports are used to increase their capacity to maintain relationships, manage service delivery tasks, live more independently, and be included in their community
- Monitor the budget to ensure providers are providing services as agreed with the participants and within the allocated budget.
- Prepare and submit reports to NDIS.
**Essential Criteria**:
- Previous experience and knowledge of NDIS and the practice standards.
- Experience in planning and coordinating meetings with clients and key stakeholders.
- Previous experience in support coordination and case management with strong clients focus.
- Demonstrated commitment to ensuring clients confidentiality.
- Strong written and verbal communication skills with experience in case notes and report writing.
- Effective problem solving, negotiation and conflict management skills.
- Ability to work with competing priorities in a busy environment and to work both independently and as part of a team.
- Excellent organisational skills with the ability to achieve deadlines in a fast-paced environment.
- Valid driver’s licence;
- NDIS Check (or willing to obtain); and
- up-to-date COVID-19 vaccinations (3 doses).
**Desirable**:
- Relevant tertiary qualification in disability community service or similar
