**About You**:
Experienced supporting people with mental health issues and cognitive disabilities you will be motivated by seeing these individuals thrive within their communities as a result of care plans that promote independence, resilience and confidence.
You are interested in case management or have had similar responsibilities in a previous role where you have supported a portfolio of clients in helping them to access supports and services within their community.
**Additional attributes we are looking for include**:
- Knowledge of accessing community resources, particularly in the areas of health, drugs and alcohol, legal, income support, employment and recreation
- Capability to advocate for others and build support networks
- An understanding of the Trauma Informed Care Network as well as NDIS Framework
- Prior experience in mental health support or working with those who are disadvantaged in the community
We are looking for someone that will take the initiative to ask questions and can work collaboratively, being comfortable that this will be a work from home arrangement. You will have a willingness to learn and ability to stay motivated while working autonomously.
It is a requirement of this position to be fully vaccinated against COVID-19, acquire and maintain a positive Working With Children’s Check, an NDIS Worker Screening Check and a current & valid driver’s licence.
About the Role
This role will provide care coordination to people we support with complex care needs. Complex care coordination is the organization of consumer care services and building relationships with other specialist services. The people we support have experienced trauma, have multiple complex needs and have been diagnosed with a mental illness and/or disability. The goal is to enable rather than do and promote independence, development and confidence of the individuals by working with relevant services.
**The role entails**:
- Arrange and attend case conferences
- Make linkages and build relationships and referral pathways to assist consumers in preparation to access options, including NDIS
- Assist to test/retest consumer eligibility for the NDIS, where consumers are assessed as needing ongoing and higher levels of services
- Links to clinical services and supports
- Knowledge of, liaise with and link to multiple external services
- Address barriers to engagement
- Establish care plans for consumers
- Support client self-management
