**Location: 104 Walker Street Dandenong 3175**
**Employment Type: Casual Contract**
**Pay Rate: $38 per hour**

**Your Role**:
As a Support Coordinator, you’ll play a vital role in helping NDIS participants implement their plans, connect with the right services, and build the confidence to manage their own supports. Your work will be key in promoting choice, control, and independence for each person you support.

**Key Responsibilities**:

- Assist participants to understand and implement their NDIS plans
- Coordinate and connect participants with suitable supports and services
- Build capacity for participants to manage their own supports
- Liaise with families, service providers, and stakeholders
- Monitor progress and assist in goal reviews
- Maintain accurate records and documentation

**What We're Looking For**:
You are:

- Compassionate, respectful, and participant-focused
- Experienced or knowledgeable in the NDIS space
- Highly organised and capable of managing a flexible workload
- A confident communicator who builds rapport easily
- Committed to empowering people to achieve their goals

**Requirements**:

- Relevant qualifications in Community Services, Social Work or similar
- NDIS Worker Screening Check
- Working With Children Check and Police Check
- Reliable transport and valid driver’s license (if field work required)
