We are looking for an experienced Team Leader to support our NDIS Support Coordination team and clients. You will be responsible for leading our NDIS Support Coordination team as well as managing a small caseload of NDIS clients. You will be leading the team to assist their NDIS participants fulfil the goals in their plans by providing assistance to identify support providers that best meet their needs. You will also support participants to link with appropriate services, organisations and activities in the community.

This is a permanent full time position, consisting of 38 hours per week and adopts a hybrid model of work.

The salary range for this position is from $89,000.90 to $97,233.37 per annum.

**Our requirements to provide care**
- Relevant tertiary qualifications inn Aged, Health or Community Services and/or Business management, Certificate IV in Leadership or working towards
- Minimum 2 years' experience in leading a team
- Significant experience within the Disability sector
- Has received three doses of an approved COVID-19 vaccine; or has been issued with a Medical Contraindication Certificate on the prescribed NSW Health form, exempting from vaccinations
- Sound working knowledge and experience in operationalising Disability Services within the community and a thorough understanding of the NDIA
- Complex case management experience
