**The role**:
We have an exciting opportunity for an Activity Coordinator experienced with NDIS plans and individual programs.
The role provides support to individuals in a variety of settings to help with developing and implementing their specific programs to empower them to live their lives as independently as possible and reach their full potential.
Social Connect aims to enrich people’s quality of life through a Person-Centred Active Support approach using every available daily opportunity to promote their:
- Participation and inclusion.
- Relationship building.
- Choice and control in all aspects of decision-making that affects them.
- Dignity and respect in all aspects of their lives.
- Skills that are used and developed to create achievement.
**To be eligible for this role, you will need to present**:
- Victorian Driver's License
- NDIS Workers Screening Check
- Working with Children Check (Victorian, Employee)
- Full COVID-19 vaccination status
- NDIS Workers Orientation Module
