**About the facility**
- Our major sites include University Hospital Geelong in Geelong’s CBD, the McKellar Centre precinct in North Geelong, Barwon Health North in Norlane, and residential aged care facilities spanning the region. Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac.
- Connected Care Program was established in 2023, working in the northern suburbs of Geelong. The inter-professional team work with clients with chronic and complex health conditions to decrease hospital demand through the provision of comprehensive assessment, care coordination and support to access specialist care in the community.. The model of care is responsive and agile to client and organisational needs The team includes care-coordinators, nurses, medical staff and allied health professionals including psychology, diabetes educator, social workers and pharmacist.

**About the role**
- As the Social Worker in the Connected Care program, you will provide vital support to clients enrolled in the service. This role includes direct client care, secondary consultations to support team members, and contributing to the development of an agile model of care that delivers excellent health outcomes. You will also play a key role in care coordination as part of a multidisciplinary team, delivering consumer-centred care through home visits, telehealth, phone, or in-clinic appointments. Based at the Staff Hub in North Geelong, you may work across other Barwon Health locations as required.
