About The Role
We are seeking a compassionate and motivated Support Coordinator to join our team. As a Support Coordinator, you are responsible for proactively assisting Deaf, Deafblind, and hard-of-hearing customers in utilising and managing their NDIS plans. You will play a vital role in ensuring that participants have access to the necessary services and supports, empowering them to live more independently and achieve their goal.
**Main responsibilities of the role include**:
Collaborate with clients to review and manage their NDIS plans, ensuring timely service delivery and effective use of available supports.
Research, coordinate, and manage a range of supports tailored to clients’ needs across multiple providers and platforms.
Promote community awareness of available services and support options.
Ensure timely and accurate completion of service agreements and related tasks, maintaining audit-ready documentation.
Track, monitor, and report hours against each client’s allocated budgets.
Maintain accurate records and progress reports on clients’ NDIS goals.
About You
Fluency in Auslan.
Extensive knowledge of, and awareness of the Deaf community and culture.
High level of numeracy and literacy skills.
Advanced Microsoft office skills, especially using excel.
Passionate about provision of high-quality person centred supports for our Deaf and hard of hearing individuals
Ability to work both independently and as a part of a team
Ability to build and maintain strong community connections and relationships with service providers, individuals and/or families/ carers.
Completed NDIS Worker Orientation and NDIS Worker Screening Check (or commitment to obtain).
You should also let us know if you have these other great skills:
Relevant qualification such as a minimum of Cert IV in Disability Services
