**About the role**:
Based in Caboolture as part of our Intercept team, the role of Case Manager works within our Reconnect program delivering holistic case management and evidence-based Intervention to support to young people and their families/carers.

In addition you will;
- Undertake initial and ongoing assessment of client needs
- Assist young people identify and pursue goals
- Undertake counselling support, advocacy, case management and mediation
- Provide sustainable interventions to stabilise and improve young people’s housing situation and improve their level of engagement with family, education, training, employment and their local community.
- Ensure all casework is underpinned by a theoretical framework
- Regularly liaise with parents/carers
- Assist in the development and facilitation of targeted individual and group programs

**About you**:
To be successful in this role you will hold a relevant tertiary qualification and equivalent experience in human service delivery.

In addition you will:

- Maintain a current working with children check (blue card)
- A current Australian drivers licence.
- Have excellent written and verbal communication skills
- Possess the ability to work autonomously
- Must hold a current NDIS Worker Screening clearance
- Intermediate Microsoft Office skills
