Case Manager - Member Engagement Officer

Come and join our amazing team to help us deliver outstanding support to our members and also build on your existing case management experience. We are looking for someone who can continue the great work of building the rapport that we have created with our members and their families.

We are the only Disability Company that received that tick from Accreditation from NDIS Audit and we are very proud of the professional services that we provide to our Members.

**Responsibilities**:

- Promotion of services (Business Development) to potential new members and their families.
- Building relationships/networking with potential members, referral sources etc.
- Actioning New Member enquiries.
- Full set up of New Members.
- Recruiting for all your members new and old.
- All scheduling of shifts to be completed by you for your members and monitored each day.
- Maintaining relationships with your members, families, and stakeholders.
- Producing budgets and service agreements for members.
- Monitoring and responding to NDIS plan changes.
- Ensuring administration procedures are adhered to.
- Keeping accurate details on members through the company’s CRM.
- Ensuring compliance under the NDIS Commission.
- Liaising with other departments on matters pertaining to members.
- Effective conflict resolution.
- Ensuring all files are always up to date and compliant.

Skills and Qualifications:

- Intermediate/high experience in Microsoft Programs.
- Qualifications within community/support.
- Minimum 3 years of demonstrated experience in a similar role or Case Management/Care coordination.
- Capacity building and skills development with people with disabilities.

**Requirements**:

- Current First Aid and CPR Certification.
- Current NDIS Workers Screening and Blue Card.
- Current valid Driver’s Licence.
- Excellent Networking Skills.
- Excellent Time Management and Organisational Skills.
- Capacity to engage constructively and respectfully with individuals of diverse backgrounds and abilities to achieve high-quality outcomes.
- Ability to work autonomously, under limited supervision, and have the ability to take the initiative, when required.

Highly Desirable:

- Strong written and verbal communication skills.
- Excellent administrative skills.
- Professional phone manner and excellent customer service.

Your Way Support Services is a Disability Service Provider offering In-Home support, and Community Access to people with Disabilities throughout the Moreton Bay, Sunshine Coast, Gympie Region and the Gold Coast.

Your Way is a certified provider under the NDIS (National Disability Insurance Scheme) and prides ourselves on working closely with our members (participants) and Support Workers to achieve their individual goals.

We are a young fast-growing company that is seeking an all-rounder Case Manager to work within our Member Services Team

About the Role:
Successful applicants will be required to undergo a current National Police Check.

**Salary**: From $50,000.00 per year

- 8 hour shift

**Experience**:

- Case management: 2 years (preferred)
