**About the Opportunity**
Located at Caboolture, this position is part of the Brisbane North PHN Team and as such there may be a requirement to work across different sites in Moreton Bay on occasion.
This is a permanent full time position working 38 hours per week.
The successful applicant will possess tertiary qualifications in a health-related discipline and be required to provide a high level of counselling support (including brief intervention) and case management to clients with complex requirements across our Caboolture AOD programs.
**Your duties in this role will also include**:
- Working with clients to develop, implement and review individual treatment goals.
- Ensuring continuity of care by effectively communicating within the multidisciplinary team and across interfaces with other health care providers.
- Providing pre and post support for rehabilitation and/or ambulatory detox clients, including relapse prevention planning.
- Providing culturally appropriate practice as relevant to varying clients.
- Ensuring that client records, data collection and reporting are consistent with confidentiality and relevant practice standard
