The position is responsible for the development, implementation, monitoring and evaluation of an integrated approach to care provided outside of the hospital setting.

Primary Objectives of the role are:

- Deliver high quality accessible care.
- Contribute as a member to the Out of Hospital Care service stream.

ISLHD operate four hospital sites on Dharawal Country in the Illawarra: Coledale Hospital, Bulli Hospital, Wollongong Hospital and Shellharbour Hospital.
